Student Pre and Post-Assessment Nutrition Surveys
- The surveys below match Dairy Council of California’s student nutrition workbook pre-assessment questions with matching questions at post-assessment.
- Page 2 is “About You” that asks for demographic information. If you do not need data by student, you may choose which questions you have students complete. Questions on all other pages are required.
- Students can view their score and check answers immediately after they complete the questions. Educators can ask students for their point total to assess knowledge.
- To access all class results, have students complete Teacher Name and School Name. Email info@dairycouncilofca.org to request the survey results be emailed to you. We welcome your feedback and suggestions on this new format. Thank you!
